Thursday, October 3 | Welcome Reception
Friday, October 4 | In-Person Preparation | 5pm-6pm
Saturday, October 5 | In-Person Dosing |12:30pm-3:30pm
Sunday, October 6 | In-Person Integration | 8am-9am
Cost to Participate: $500
You will be required to save a credit card on file with us to be charged after you’ve completed your medical intake. If you are interested in participating but have financial barriers, please let us know and sliding scale/scholarship options will happily be explored further.

3. Medical Intake

4. Clinical Intake
Sign ups are now closed.
5. Blood Pressure Reading
Send a recent blood pressure reading (photocopy or picture) to sstroeh@discoverthemanor.com, erincurtismd@gmail.com, and AbbyM@integratahealing.com.
6. Forms & Measures
You will be added to our practice management system, Owl. Look for two emails from notifications@owlpractice.com with the subject lines “Integrata - Forms” and “Integrata - Clinical Measures” to complete ahead of your intakes.
7. Billing
Create a Client Portal account HERE and save a credit card on file. Your credit card will be charged $500 by Integrata’s team after you complete your medical intake. If you do not move forward with participating for any reason, you will only be charged a portion of the participation fee to cover any completed intakes.
8. Event Schedule
Attend in-person welcome reception Thursday (10/3), small group preparation Friday (10/4), small group dosing Saturday (10/5), and small group integration Sunday (10/6).
