KAP Participants
Please make sure you’ve scheduled time to meet with both Dr. Erin Curtis and Dr. Chantelle Thomas.
If you are interested in being a KAP participant, we ask that you meet with Dr. Curtis first if possible. Her scheduling link is here.
This scheduling order is preferred but not required. If you need to meet with Dr. Thomas first based on availability, that is ok. Her scheduling link is here.
You will be added to our practice management system, Owl.
We will use this system to send you forms to fill out ahead of your meetings. These will be sent to you via email from notifications@owlpractice.com with the subject line “Integrata - Forms”.
You will receive:
Intake/Demographics Form
Medical Screening - must be completed before any of your scheduled meetings
Therapeutic Group Experiential KAP Informational Materials
KAP Informed Consent
Please create an account on our Client Portal. Click “Create Account” in the top right corner of the homepage and follow the instructions to set up your account.
Once you’ve created an account, click “My Account” in the top right and under “Account Summary” please add a credit card to your account.
The credit card you save will be charged on the Friday after the group experiential takes place.
If you are interested in participating but experiencing any financial barriers, please reach out to Dr. Thomas directly, and sliding scale/scholarship options will happily be explored further.
Facilitators

You will receive a Stripe payment link after the group experiential takes place from Claire Stark, who manages Integrata’s billing.
